PARTICIPANT RELEASE STATEMENT: I realize the activity that I participate in involves risk of injury and I do hereby assume that risk.  I do hereby release, discharge and hold harmless the City of Kearney, its agents, and employees from actions, causes of action, claims, or demands of any kind which may result from my participation in this activity.  I agree that I will be barred from pursuing any claims against the City of Kearney which may result from my participation in this activity. 

PHOTO/VIDEO PERMISSION:  We, the parents or participating individual, do hereby grant permission for pictures and videos to be used in publicity or brochures related to the City of Kearney Park and Recreation Program. 

SPORTSMANSHIP: A high level of sportsmanship is the standard.  If applicable, I have read and understand the sportsmanship requirements, set forth in the rulebook for the activity I am participating in.  I agree to adhere to these rules and agree to the discipline procedure if needed.
