I (We), the Undersigned, for and in consideration of the privilege to make use of the Walton Life Fitness Center, completely and fully release Walmart, its subsidiaries, assigns and successors from any and all li- ability, claims, actions, causes of action, demands, damages, costs, expenses and compensation whatsoever that may arise from my (our) participation in any activity at / or sponsored by the Walton Life Fitness Center at any time. I (We) intend that this release apply to any and all injuries, damages, losses and expenses, past or future, which may arise from the use of the facilities of the Walton Life Fitness Center or my (our) participation in any activity sponsored by the Walton Life Fitness Center.

Furthermore, for and in consideration of the privilege for my (our) minor child (children) to make use of the Walton Life Fitness Center, I (we) affirm that I (we) have legal custody of said minor(s) and understand that on behalf of the minor(s), myself and all other legal guardians, my heirs, assigns, and legal representatives, that I (we) hereby completely release Walmart and its subsidiaries and successors, from any and all liability, claims, actions and causes of action, demands, damages, costs, expenses and compensation whatsoever in the event that said minor(s) should be injured in any manner while participating in an activity, and will hold Walmart harmless in the event said minor(s) shall cause any damages or injury to property or persons while on or about the premises of the Walton Life Fitness Center.
